Procedure For Filing A Stolen US Online Passport

A stolen U.S. online passport requires following a list of legal procedures including the reporting of the theft and an application for a new one. The legal document is issued by the United States Embassy and is necessary if you want to be able to travel abroad. One has to complete a number of designated forms should you experience the event of theft of this essential travel document.

Step 1

The very first requirement in a case where the document has gone missing is to report the incident to relevant authorities. If the document was taken while in the United States, you are advised to contact the Department of Services in the United States for passports to invalidate the item. Individuals who are out of the country must seek the services of the Embassy or Consulate.

DS-64

The DS-64 form is usually required to be completed and must be done as soon as possible. The specific form is necessary in recording your statement, which will document that it has been lost as a result of theft. The alternative to filling out the necessary documentation is to contact the Passport Services of the United States that will invalidate the document.

Invalidation

In instances where the travel booklet has been invalidated and is later found, it cannot be re-validated but must be sent to the State Department for destruction. Individuals traveling internationally will need to find the nearest U. S. Consulate or Embassy. The DS-64 form must be filled out and the State Department notified of your loss.

Accuracy

Always proofread all items that have been filled out and correct any misspelled information as this will prevent delays and denial in processing the application. Often, a police report also has to be completed and submitted together with application for a replacement booklet when submitting the necessary paperwork. This report is necessary for proof of the theft and details the events surrounding its disappearance.

Loss of a Valid Passport

If your legal document was valid at the time that it went missing, the DS-11 and DS-64 forms will have to be completed and submitted together. This is because one will have to apply for a new legal document, which is the reason that the DS-11 form is needed to be completed and delivered to relevant authorities. A number of specific documents are necessary when applying to replace the important travel document.

Identification

You are required to produce proof of birth certification or naturalization. It is also necessary to include proof of identification in the form of a driver license or government issued card and two passport size photographs. A fee will be charged for the application process and if you have pressing travel arrangements, stolen passport services are available at an extra charge.
